Victoria.
We are an SIL and SDA provider who are seeking a NDIS Case Manager to join their fast growing team based in Melbourne’s Inner Northern Suburbs.
As a Case Manager, you will play a crucial role in supporting individuals with disabilities or
mental health conditions in their journey towards independent living. Your primary
responsibility will be to provide comprehensive case management services, coordinating and
advocating for the well-being and success of our clients.
About the role:
- Conduct initial assessments to determine the individual needs, goals and suitability of
clients entering the Supported Independent Living program.
- Develop and implement personalized service plans in collaboration with clients, focusing
on their strengths, preferences, and desired outcomes.
- Coordinate and facilitate access to appropriate community resources, including medical,
mental health, employment, education, and social services.
- Provide ongoing support and guidance to clients, promoting their independent living
skills, self-advocacy, and overall personal growth.
- Monitor clients' progress and make necessary adjustments to service plans, ensuring
continuous improvement and attainment of goals.
- Collaborate with interdisciplinary teams, including healthcare professionals, therapists,
social workers, and family members, to ensure holistic and coordinated care.
- Advocate for clients' rights and needs within the community, addressing any barriers
they may encounter.
- Maintain accurate and up-to-date documentation of client interactions, progress, and
service provision, adhering to privacy and confidentiality guidelines.
- Participate in regular team meetings, case conferences, and professional development
opportunities to enhance your knowledge and skills.
Skills & Experience:
- Previous experience working in SIL NDIS provider as a Case Manager
- Ability to work autonomously
- Highly developed time management, planning and organisational skills
- Knowledge of disability services, mental health, and relevant community resources.
- Strong assessment, planning, and problem-solving skills to develop individualized
service plans.
- Knowledge in SIL and SDA
- Excellent communication and interpersonal skills to effectively collaborate with clients,
families, and multidisciplinary teams.
- Empathy, patience, and the ability to provide nonjudgmental support to individuals
facing diverse challenges.
- Valid driver's license and access to reliable transportation
