Primary Care Mental Health Clinical lead B7: Coastal Primary Care Network (PCN)

Coastal PCN we looking for a dynamic individual to take over the development of a brand-new model as part of the transformation of mental health care in Cornwall. There has never been a more exciting time to work in mental health services, especially in primary care. We want to hear from people who have passion and creativity to help shape and build our new, holistic service in Coastal PCN. We aim to create a space where we understand ‘what matters to someone’ not ‘what is the matter with someone’. Our new team will include a diverse range of people all there to listen, triage and hopefully help with aspects of a person’s life that may adversely affect their mental wellbeing.

Due to these positive & transformative changes, we now have an exciting and unique opportunity for the right person to lead this transformation within the Coastal Primary Care Network under the employment of Cornwall Foundation Trust (CFT). Our PCN has a population of over 28,000 people set on the beautiful south coast of Cornwall, incorporating 4 practices in close proximity, with excellent inter-relations and a long history of working together. Our population is diverse compared to other areas in Cornwall, with varying levels of socio-economic status, young families, and elderly patients all of whom make providing care very interesting.

Main duties of the job

The aim is to create a more sustainable and resilient PCN servicing the needs of its patients and ensuring the best possible care. The model in which we operate is around creating closer integration with community nursing teams, social prescribing and coordinated frailty management. We are committed to developing a team that trusts each other so that we can innovate and encourage continual improvement. We believe in patient-centred care that is meaningful, personalised, and valuable to each individual patient. This includes listening to, informing and involving patients in their care each step of the way. This will enable patients to effectively manage and make informed decisions about their health and care achieving better outcomes.

The individual surgeries regularly appear in the Top Ten Practices in the county based on the latest findings from the ‘GP Patient Survey’ - a huge ongoing national poll involving more than a million people a year. The survey covers everything from the trust patients have in their doctor, to the attitude of reception staff and how easy it is to get an appointment

Person specification

Education / Qualifications and Relevant Experience

Essential criteria
  • Relevant professional qualification e.g. Dip OT, Dip SW, RMN, Clinical Psychology Grade A, or equivalent
  • Experience of working in a mental health care setting and of liaising and working with GPs
  • Suitably trained and experienced in the delivery of short term, focussed evidence-based interventions
Desirable criteria
  • non medical prescriber
  • Further qualification in mental health work
  • Willingness to undertake further study relevant to mental health in general practice (for example independent prescribing), supported by GP clinical supervision.

Skills and Aptitude

Essential criteria
  • Receptive to and able to use clinical supervision appropriately
  • An appreciation of the nature of GPs and general practice. An appreciation of the nature of primary care prescribing.
  • Experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice
Desirable criteria
  • Understanding of the implications of national priorities for the team and service
  • In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
  • Experience of handling clinical incidents and complaints
